This Privacy Policy statement is issued by Radiant Interactive Group, Inc. (hereafter "Radiant") and those companies which licenses Radiant's website properties (hereafter "Licensees").

This Privacy Policy document applies to all Websites owned and operated by Radiant and licensed to Radiant’s Licensees.

The Websites are hereafter collectively referred to as the “Websites”.

This notice describes how information about you is gathered, used, and disclosed, and how you can get access to this information. Please review this notice carefully.

Introduction
Radiant and its Licensees are committed to safeguarding your privacy on our Websites. Federal law requires health care providers, such as mental health practitioners to maintain the privacy of protected health information and to provide notice of their legal duties and privacy practices with respect to such health information. Some state laws are even broader than Federal law and require that other organizations, including organizations such as Radiant, maintain the privacy of protected health information and provide notice of their legal duties and privacy practices with respect to such health information. In some instances, Radiant may collect and maintain protected health information on your behalf for the benefit of mental health practitioners with whom you contract, and is therefore also bound by certain duties. Because trust is such an important part of this relationship, Radiant has established this Notice. This Notice is intended to provide you with information about your rights and Radiant's responsibilities under the law, as well as information about Radiant‘s privacy practices. Radiant is required to abide by the terms of this Notice.

Scope of This Privacy Policy
The terms of this Notice apply only when you are operating within the Websites and to information collected and maintained by Radiant. The terms of this Notice do not apply to web sites that you may access via links on the Websites. Radiant is not responsible for the content or the privacy practices of other web sites. If you disclose personal information to other web sites, you are subject to the other web sites' privacy policies. We encourage you to review a third party web site's privacy policy before disclosing any personal information.

What Information Do We Collect?

Registration Information. To utilize certain services in the Websites, you will be required to provide your name, email address, and the city and state you reside in.

Member Profile. If you establish an account you will be required to create a profile on the site in which you will post certain personal information such as your age, gender, relationship status, number of children, city and state of residence and your zip code which will be viewable by all members and affiliated practitioners. To protect your identity, however, you will be required to adopt a username or “handle” other than your real name, which will be posted to your profile.

Photo Uploads. You will be given the option of posting a photo of yourself to your member profile.

Personal Journal. You will be given the option of maintaining a personal journal on the Websites which will be maintained in a secure area, and viewable only by those members and/or practitioners to whom you grant access.

Cookies. Radiant also collects information about your use of its Websites through the use of cookies. Cookies are small pieces of information passed by the web server to your browser. We use cookies to identify you to the web server during page requests and to keep your session alive. You can control the use of cookies through your browser settings.

How Does Radiant Use and Disclose Your Information?

Registration Information. Radiant does not disclose your name, or email address acquired during the registration process to anyone.

Member Profile. Your member profile is viewable by all site members and affiliated practitioners. However, you will select and post a username or “handle” to your profile in lieu of posting your real name, so as to maintain your anonymity. Radiant will never disclose to any other member, practitioner, or other third parties, your real name, address, email address, or other relevant information about you, except as may be required by law.

Photo Gallery. Posting a photo to your profile for all members to see is strictly voluntary, and if posted, is done by you and not by Radiant.

Personal Journal. Posting of your personal journal to the site is strictly voluntary, and in all cases is stored in a secure area. Your journal, if applicable, may only be viewed by those members and/or practitioners to whom you grant access. Radiant does not publish, release or give access to your journal to anyone, unless required by law to do so.

Postings to Forums. Any postings you make to the Q & A Forums will appear under your username only. Radiant will never disclose or publish your real name, or reveal your identity whenever you place postings to its forums, unless required by law to do so.

Opt In Email. Upon creating an account on any of the Websites, you are automatically included in our opt-in email list. This means that you will periodically be sent emails, newsletters, announcements, and advertisements from Radiant. Radiant will not sell or provide your email address to anyone, but may send you correspondence containing offers and information from third parties. All correspondence sent to you by Radiant will contain a “Remove Me” link to enable you to opt out of future mailings, if you so choose.

Aggregate Information. Radiant may make available to unrelated third parties aggregate information about you. Aggregate information is statistically relevant information based on the overall population of users of the Websites and does not identify the particular individual whose information has been aggregated.

Future Management. If Radiant becomes owned or controlled by other individuals or entities, the information we have collected may be transferred to such party or parties. Such individuals or entities will be required to follow the terms of this Notice in the place of Radiant's current owners.

Advisory. If you elect to post a personal journal or photos to the site, you are advised to use extreme discretion in whom you grant access to view these postings. Radiant and its Licensees shall not be responsible nor liable for any information which becomes public, or is made available to unintended persons relative to these postings as a result of your decision to grant access to these postings to other site members or practitioners.

Disclaimer. Although Radiant will use all reasonable efforts to safeguard the confidentiality of your personal information, transmissions made by means of the Internet cannot be made absolutely secure. Radiant will not be liable for disclosures of your personal information due to errors in transmission or the unauthorized acts of third parties.

Uses and/or Disclosures NOT Requiring your Authorization.

We may use and/or disclose your information without your authorization in the following circumstances:

When required by law: We may use and/or disclose your private health information (PHI) when existing law requires that we report information including each of the following areas:

Reporting abuse, neglect or domestic violence: We may use and/or disclose your PHI of suspected victims of abuse, neglect, or domestic violence including reporting the information to social service or protective services agencies.

Public health activities: We may use and/or disclose your PHI to prevent or control the spread of disease or other injury, public health surveillance or investigations, reporting adverse events with respect to food, dietary supplements, product defects and other related problems to the Food and Drug Administration, medical surveillance of the workplace or to evaluate whether or not you have a work-related illness or injury, in order to comply with Federal or state law.

Health oversight activities: We may use and/or disclose your PHI to designated activities and functions including audits, civil, administrative, or criminal investigations, inspections, licensure or disciplinary actions, or civil, administrative, or criminal proceedings or actions, or other activities necessary for appropriate oversight of government benefit programs.

Judicial and administrative proceedings: We may use and/or disclose your PHI in response to an order of a court or administrative tribunal, a warrant, subpoena, discovery request, or other lawful process.

Law enforcement activities: We may use and/or disclose your PHI for the purpose of identifying or locating a suspect, fugitive, material witness, or missing person, or reporting crimes in emergencies, or reporting a death.

Relating to decedents: We may use and/or disclose the PHI of an individual’s death to coroners, medical examiners and funeral directors.

For research purposes: In certain circumstances, and under the supervision of an Internal Review Board, we may disclose your PHI to assist in medical/psychiatric research.

To avert a serious threat to health or safety: We may use and/or disclose your PHI in order to avert a serious threat to health or safety.

For specific government functions: We may use and/or disclose the PHI of military personnel and veterans in certain situations. Similarly, we may disclose the PHI of inmates to correctional facilities in certain situations. We may also disclose your PHI to governmental programs responsible for providing public health benefits, and for workers’ compensation. Additionally, we may disclose your PHI, if required, for national security reasons.

What rights do you have over the information we maintain?

You have a number of important rights with respect to the information held by Radiant on your behalf. You have a right to inspect and copy protected health information that Radiant maintains about you and to receive an accounting of certain uses or disclosures of your information. You have the right to request to receive your health information on a confidential basis through certain locations or means (e.g., only contact you at work or through the mail). You not only have a right to update your own personal health information, but you also have a right to request amendments or corrections to any health information held by Radiant that is incorrect or incomplete. In such case, you will either amend your information yourself as such information may be amended by you on our Websites, or you will need to contact your mental health practitioner and follow your practitioner’s procedures regarding the amendment of your health information. You also may ask Radiant to further restrict, beyond the terms of this Notice, how your health information will be used or disclosed. Radiant will consider these requests, although Radiant is not required to agree to such further restrictions. You also have the right to receive a paper copy of this Notice. How we protect your information?

Any information provided to Radiant is treated with care and discretion. Radiant uses all reasonable efforts to keep information from our users as secure as possible against unauthorized access and use. When external entities are provided information in accordance with this Notice, Radiant will, in good faith, make an effort to ensure that the third party complies with the privacy policies of Radiant.

As part of the registration process, your account is password protected. Radiant encrypts your password on its database. It is important for you to protect your password and computer from unauthorized access.

When you submit personal information to the secure areas of the Radiant Websites, we encrypt it using Secure Sockets Layer ("SSL"), an encryption software program. Encryption is the industry standard means to protect information as it passes over the Internet to our server. We use care to protect this information against unauthorized access and modification once it is stored on our database.

Children's Privacy

The Websites are not directed at children. Radiant will take reasonable steps to ensure that it does not collect personal health information from children under the age of 18 (though parents may, in their discretion, provide information about their children, and this information will be safeguarded in the same way as other health information).

How will you be notified of future changes to this Privacy Policy?

While our commitment to protecting your privacy is steadfast, from time to time we may change or update this Notice. When significant changes are made that affect how your information is used, we will post a notice on our home page, or alert you to changes by other similar means.

You will have a choice as to whether or not we use your information in this different manner. By continuing to use our Websites, after choice has been given, you agree to be bound by any such revisions and should therefore periodically visit this page and print the latest version of the Notice for your records. Radiant will apply the latest version of the Notice to all information that it holds, regardless of when Radiant came into possession of the information. For your information, the date of the last update to this Notice is stated at the top of the document.
