How It Works
You create a graphic or widget on your website home page where your site visitors can access the Eldercare Screener (View Sample).
Your site visitors who click on the graphic/widget to take the screening will be redirected to the Eldercare Screener’s 12 item Assisted Living Questionnaire. The questionnaire accurately identifies those who are candidates for assisted living, while filtering out those who are not (see questionnaire)
Screenings can be self-administered by individuals who want to know if they need an assisted living arrangement, or by friends or family members on behalf of a loved one about whom they are concerned.

Additional Marketing Opportunities
In addition to making the Eldercare Screener available to your website visitors, you can also use it to build a marketing campaign to encourage public screenings and drive traffic to your website. Links to the Eldercare Screener can easily be embedded into any internet-based display ad, pay-per-click ad, or print ads.

For scores ranging between 0-5:
Assisted Living is not indicated at this time. However, watchful waiting is recommended. Feel free to take this questionnaire in the future, particularly if you notice any immediate or significant change in the areas of functioning covered by this questionnaire.
For scores ranging between 6-10:
Assisted Living is not indicated at this time. However, it is recommended that you re-take this screening in 6 months, or right away if you notice any immediate or significant change in the areas of functioning covered by this questionnaire.

For scores ranging between 11-20:
Assisted Living may be indicated.
For scores 21 and above:
Assisted Living is indicated. Depending upon the degree of need, access to skilled nursing may also be indicated.
Below is a sample graphic that would be installed by your webmaster on your website home page to encourage your site visitors to take a screening. Note that this sample graphic includes a $500 admissions credit as an incentive to get visitors to complete the screening. This is just a suggestion. You can create any incentive you wish, or none at all.
You can select which addictions to screen for, and only those screeners you select will be displayed to your website visitors
Each time one of your website visitors takes a screening and screens positive for an assisted living arrangement, they are given the option to share their screening data with, and request contact from, those of your staff members who have created an account on the Eldercare Screener administration site. The screening data and contact request park in the Contact Requests inbox. Staff members can log in any time to the admin site from any internet-connected device to retrieve the screening data and contact requests from the inbox.
Each time screening data, and a request for contact is received from one of your website visitors, your staff members who have created an account on the Eldercare Screener administration site will be notified via email of the pending request. This in turn will prompt them to log in, review the screening data, and make contact with the prospect.
This feature allows a staff member who has access to the admin page to "capture" a prospect. Upon doing so, all other staff members with admin rights will be notified for the capture via email. This helps avoid more than one staff member attending to the same prospect.
